To give our patients the best care, we need to understand how the elbow works. This includes the nerves, muscles, and bones that make it up. Let’s start by talking about nerve injuries because they’re a big part of elbow problems.

Reviewing Nerve Injuries for Better Treatment Decisions
Remember, nerves are our body’s communication highways. They carry messages from the brain to our muscles, telling them to move, and relay sensory information back to the brain. When a nerve gets injured, this communication gets disrupted, leading to a variety of symptoms like pain, weakness, numbness, and tingling.
There are three main types of nerve injuries:
- Compression: This occurs when something presses on a nerve, like a bone spur or inflamed tissue.
- Traction: This happens when a nerve gets stretched or pulled too forcefully.
- Laceration: This is the most severe type, where the nerve is completely severed.
As hand therapists, we primarily deal with compression injuries, which are often chronic in nature. These can significantly impact elbow function, making a thorough evaluation crucial.

Does the patient experience any numbness, tingling, or weakness in the forearm or hand?
Previous Injuries or Medical Conditions
Has the patient had any previous injuries to the elbow or other medical conditions that could be contributing to the pain?
These questions, along with a thorough physical examination, will help paint a clearer picture of the underlying cause of the elbow issue.
Unraveling the Mystery: Specific Tests for Elbow Evaluation
In addition to the questions above, we often utilize specific tests to pinpoint the root of the problem. These tests may include:
- Nerve Conduction Studies (NCS): This test measures the electrical activity of nerves to assess for nerve damage.
- Electromyography (EMG): This test evaluates muscle function by measuring the electrical activity of the muscles.
- X-rays and MRIs: These imaging studies can reveal bone fractures, ligament tears, and other structural abnormalities in the elbow joint.

Putting the Pieces Together: Crafting a Treatment Plan
By analyzing the answers to the evaluation questions and the results of any additional tests, we can develop a personalized treatment plan. This plan might include:
Manual therapy: Techniques like massage and joint mobilization can help improve the range of motion and reduce pain.
Splinting: Splints can be used to immobilize the joint or to stretch tight tissues.
Therapeutic exercise: Specific exercises can help strengthen muscles, improve flexibility, and restore normal function.
Modalities: Ultrasound, electrical stimulation, and other modalities can be used to reduce pain and inflammation.
In some cases, surgery may be necessary to address severe nerve damage or structural abnormalities.
Beyond the Basics: Common Elbow Injuries and Their Challenges
Now that we’ve established the foundation for evaluating complex elbow injuries, let’s dive deeper into some common conditions and the unique challenges they present:
Lateral Epicondylitis (Tennis Elbow)
This condition is characterized by pain on the outside of the elbow, often caused by overuse of the wrist extensor muscles.
Medial Epicondylitis (Golfer’s Elbow)
This condition involves pain on the inside of the elbow, typically due to overuse of the wrist flexor muscles.
Cubital Tunnel Syndrome
This condition occurs when the ulnar nerve, which runs along the inside of the elbow, becomes compressed.
Radial Tunnel Syndrome
This condition involves compression of the radial nerve, which runs along the outside of the elbow.
Elbow Dislocations
These injuries involve the bones of the elbow joint becoming dislodged from their normal position.
Elbow Fractures
Fractures can occur in any of the bones that make up the elbow joint.
Each of these conditions has its own set of symptoms, diagnostic tests, and treatment options. As a hand therapist, it’s essential to be well-versed in these conditions and their nuances.
Case Study: A Complex Elbow Injury
Let’s imagine a patient who’s a construction worker. He’s been having elbow pain for a few months and finds it hard to lift things or hold onto tools. When we examine his elbow, it’s sore on the outside, he can’t move it as much as he used to, and his wrist is weak.
We think it might be tennis elbow, but we want to make sure it’s not something else like radial tunnel syndrome or a nerve problem. So, we do an MRI and some tests to check the nerves.
The MRI shows that the muscles on the outside of his wrist are inflamed. The nerve tests are okay, which means it is a tennis elbow.
We make a plan to help him. We tell him:
- to rest
- avoid doing things that make the pain worse
- we give him medicine to reduce inflammation,
- exercises to strengthen his wrist
- and a splint to support his elbow.
After a few weeks, his pain is much better.
Additional Considerations: The Role of Occupation and Lifestyle
When we look at elbow injuries, it’s important to think about what the patient does for a living and how they spend their free time. Some jobs and hobbies can put extra strain on the elbow.
For example, people who do a lot of overhead work, like painters or construction workers, might be more likely to have elbow problems. Also, people who play sports that use the wrist a lot, like tennis or golf, can be at risk.
Knowing what the patient does can help us figure out what’s causing the problem and make a treatment plan that’s right for them.
To figure out what’s causing a complex elbow injury, we need to do a few things: ask the patient questions about their pain, examine them, and do some tests. By doing this, we can find out what’s wrong and make a plan to fix it.
